All students are required to have a current athletic physical packet to participate in a sport.  Students may make arrangements through their family physician or a walk in clinic and return the signed forms to the Clark Intermediate Athletic Office.

All forms must be completed and signed by student and parents to participate in athletics.INCOMPLETE PACKETS WILL NOT BE ACCEPTED IN THE ATHLETIC OFFICE.
